Have a qualifying Madico product professionally installed between October 15, 2009 and
December 15, 2009 and qualify for $50 via mail-In rebate. Qualifying product installations must
be located in the 50 United States, the District of Columbia or Canada. This offer IS for end-user
customers only. Reseller purchases do not qualify for this offer
PRODUCT ELIGIBILITY: All standard Madlco architectural window film installations over $500.00
qualify for this offer
RESTRICTIONS:
Limit 1 per address. per household. This offer is valid only in the US and Canada.
PROOF OF PURCHASE:
Submit proof of purchase consisting of a copy of your dated sales receipt showing all
products installed the installing dealers name and address; the qualifying products; and the full
retail price paid for the qualifying Installations as well as your completed warranty form and
completed rebate form (Rebate Form given to customer after purchase)
REBATE SCHEDULE: $50 rebate for a qualified installation
CLAIM PROCEDURES: Claims must be submitted via mall-in rebate. Rebate coupons may not be
altered. Duplicate or incomplete coupons will not be honored. All rebate documents must be
postmarked by January 15 2010 Rebates with postmark dates after January 15, 2010 will not
be processed. The address on your rebate application must match the billing address on the
installation receipt
